With ever-changing environments look at the business today as if you had your desired level of clients, whether 5000 or 500.000 or even 1 million look at the systems and technology which will support that, always assume you will reach your goal and think that way, everything from the colour of the walls  down to which logo you have can change, but think about how in the early days you can minimise costs of changing all of this in the short term, and yes be prepared to upgrade, overhaul your entire IT environment in 5-7 years, but look at what brands and systems are going to make sure you and your healthcare business make it that far, we live in a world of “Buy cheap, Buy Twice” when looking at our historical data clinics who chose the least sustainable infrastructure ultimately paid for the most sustainable one, they just went through years of slow systems, data issues, and ultimately lost revenue.
